Story: Medication Error
Nurse Ms. M was administering medication to a cardiac patient who was supposed to receive Tab. Amlong, 5 mg, at 8 AM. However, she mistakenly administered Tab. Atorva, 5 mg, which was scheduled for bedtime. After realizing the mistake, she immediately informed the doctor. The issue was corrected by withholding the night dose of Atorva.
Moral of the Story: Timely recognition and reporting are essential. Always double-check look-alike or sound-alike drugs and verify timings before administering.
Story: Incorrect Patient Information
During an emergency coronary angiogram, Nurse Ms. V received a serology report indicating the patient was HBsAg positive. She promptly communicated it to the OT nurse, and extra precautions were taken. Later, the printed report showed the patient was actually HBsAg negative, leading to unnecessary costs and accountability issues.

Story: Missed Treatment
A patient with abdominal distention was prescribed insertion of a flatus tube three times daily. Nurse Ms. K, on night duty, diligently performed and documented the procedure. Later, another nurse admitted she had not done it during her shift. The consultant questioned Ms. K, and the patient confirmed that only she had consistently provided the treatment.
Moral of the Story: Consistent documentation, inter-shift communication, and patient verification are crucial to ensure accountability and continuity of care.
